Team Application

July 10-12, 2010 (Saturday-Monday)

July 13-17, 2010 (Tuesday-Saturday)

» Applications will close as age brackets are filled; first come, first served.
» Team fee and dorm deposit, if applicable, are payable at time of application by Visa or MasterCard only.
» The application will take approximately 5-10 minutes to complete. Be prepared with your coach’s name, address and contact information.
» A receipt will be issued upon completion. This is your receipt for reimbursement, if applicable.

» Application deadline is July 1, 2010.
» Volunteers working eight hours or more receive a unique staff shirt that serves as both a uniform and provides access to many tournament activities.
» Volunteers 19 years old and younger will also receive a participant wristband for access to Schwan's USA CUP activities. Note: Athlete Party and dances require chaperones.
