NSC Liability Parents Waiver: |
1. Complete the tournament roster online using our Team Management tools
- » Preliminary roster is due June 15. All players, including guest players, must be listed on the tournament roster. Enter the player name, position and unique jersey number. Remember: there is NO jersey sharing. The player address, birth date and college coaches recruiting information (ages U16-U19), will be completed by the parent utilizing the online waiver. The information will automatically populate your tournament roster, making your job that much easier.
- » Addresses required; addresses are directly linked to the NSC Liability Waiver as required by the State of Minnesota.
- » Roster changes may continue until the following dates:
- » Friday, July 9 at 0900: Schwan’s USA CUP Weekend
- » Monday, July 12 at 0900: Schwan’s USA CUP
- » Final rosters will be printed on these dates/times for Final Team Check-in and will not reflect player addresses.
- » If a player must be added after these dates/times, print an updated roster and bring it to Final Team Check-in.
- » Players dropped at Final Team Check-in will receive a refund of the player fee following the tournament.
- » Players 16 and above may participate in the College Coaches Recruiting Program.
- » Parents may provide the player email address, Grade Point Average (GPA), graduation year, and SAT or ACT scores on the online waiver for the roster.
- » At a minimum, the player’s email address and graduation year are requested by the coaches for participation.
- » Players 15 and younger will leave these fields blank
2. Direct parents to complete the online NSC Liability Waiver
- » Provide parents with the tournament ID number for this task. Players are not permitted to participate in the tournament without a completed NSC Waiver on file.
- » You may monitor the progress of your parents by selecting View Roster from the online roster menu. The WV column stands for Waiver. Each time a parent completes the online waiver, an ‘X’ is displayed in this column to the left of the player’s name.
- » Waivers are tracked to each player’s address, not the parent’s email address. Therefore, a parent may complete the online waiver from any computer!
3. Approved Permission-to-Travel and Provincial/Club Team Roster.
- » Contact your state/province/club for permission-to-travel and roster.
- » Include the MYSA and US Club Soccer sanctioning located at the top of the page with your Permission to Travel.
- » Permission-to-Travel must be on file with the tournament prior to arrival. See mailing address and fax number above.
4. Guest Players
- » Canadian and United States Guest Players: Direct your guest players to contact their State Association/Canadian province/club for individual permission-to-travel or a guest player loan form. Guest players will not be permitted to play without this information on file with the tournament.
- » Guest Player Limit: Up to 6 guest players may be rostered per team.
- » No more than 2 players per team may play on one other team in addition to their primary team.
- » Email registrar@usacup.org with the names of your two double rostered players and their primary and secondary tournament ID numbers. The player fee on the secondary team will then be waived within each tournament only.
- » Players dropping from the team roster prior to the start of the first game will receive a refund of the player fee only.
5. Player passes are required for play
- » Current youth player passes are required.
- » Canadian booklets/provincial player cards and FIFA-affiliate player passes are accepted by the National Sports Center for non-MN guest players.
- » Signature must match name on card.
- » Player passes must be laminated.
- » No copy of a player pass is accepted.
- » Player passes will be presented at Final Team Check-in and at the start of each game.
6. Player fees are payable by Visa, MasterCard, check, money order or cash. Team payment only; no individual payments.
- » Player fees are determined by the number of players on your team.
- » Use the Payment/Balance Due section of the Team Management Tools to make a Visa or MasterCard payment.
- » If you are paying by check or money order:
- » Make payment payable to either Schwan’s USA CUP Weekend or Schwan’s USA CUP.
- » Record your tournament ID number on check or money order.
- » Mail payment to 1700 105th Ave NE, Blaine, MN 55449
- » If you are paying by cash, office hours are Monday-Friday, 0900-1700. The main office is located in the Administration Building on the NSC campus; enter the building from parking lot B off Davenport Street, go to plaza level.
- » Players dropped from the roster at Final Team Check-in will receive the player fee refund following the tournament.
- » No invoice will be sent to your team.
7. Required hotel lodging
- » All teams are required to stay in the USA CUP Official Host Hotels. See lodging.
- » Provide the hotel name, city and phone number; update as needed.
- » Hotel contact information is used in an emergency.
- » The National Sports Center is also required by the State of Minnesota to track for economic impact. Did you know that the National Sports Center is second only to the Mall of America for economic impact and the number of annual visitors?
8. Dormitory lodging
- » Provide the exact number of players and the number/gender of the chaperones by June 1.
- » Dorm assignments will be posted on the Team Management tools by June 7. Specific dormitory rooming list and dormitory information packet available at this time.
- » Complete the dormitory rooming list by June 13.
- » Any additions to the housing list after June 13 are not guaranteed.
- » Individuals dropping from the housing list July 1 will not be refunded the housing fee; the player fees will be refunded following the tournament.
9. Transportation Request - Dormitory Teams Only
- » Complete the Transportation Request, if your team requires complimentary transportation at the beginning/end of the tournament.
- » Update the Transportation Request as changes occur in your travel schedule.
10. Print the rules. Coaches are responsible for knowing the rules in advance. Express concerns prior to the start of each game.
